If you have any doubt on what should be disclosed in 
this Proposal Form, please contact Bank of China Group 
Insurance Company Limited (named below as “BOCG 
Insurance”) hotline (852) 3187 5100 for the interests 
of the proposed Insured Company/Insured Person. Failure 
to disclose may mean that the policy will not provide 
the proposed Insured Company/Insured Person with the 
coverage required, or may invalidate the policy altogether.
3. 若此投保书所含的内容与保单条款有任何歧异，概以保单為準。
In the event that the information contained in this 
Proposal Form does not conform to the terms in any 
policy issued, the policy terms shall prevail.
4. 「零售综合险」(下称“本计划”) 由中银集团保险承保。
Retailer Comprehensive Insurance (named below as “this 
Plan”) is underwritten by BOCG Insurance.
5. 中国银行（香港）有限公司、南洋商业银行有限公司、集友银行有限公司、中银信用卡(国际)有限公司及其他代理银行（各称為“代理银行/代理”）以中银集团保险的委任保险代理身份分销本计划，本计划為中银集团保险的產品，而非代理银行/代理的產品。
