Part 1 - Applying for Universal Life 
1.1 Eligibility
Eligible for Life Insured possess Investable 
Assets not less than HKD 1 million. Investable 
assets include cash and bank accounts plus 
securities and investment accounts that can be 
readily converted into cash. Random financial 
check will be conducted by requesting solid 
proof from client. 
1.2 New Business Requirements  
The following are required forms and documentation 
for Universal Life application. The forms 
are available on the High Net Worth Section 
of Manulife’s website for distributors.    
1.  Completed Application Form  
2. Signed proposal 
3.  Proof of identity on both Life Insured 
and Policyowner. Must be copy of Hong Kong or 
Macau ID card or passport.  
4.  Residential Address Proof of Policy 
Owner (e.g. copy of bank statement , utility bills in last 
3 months 
5.  Payment Instruction Form (ASC8) – 
applicable to payment by cheque, not applicable if 
payment by Telegraphic Transfer (TT) 
6.  One monthly Early Lapse Protection Premium. 
(If the Policy Year Date is prior to the issue 
date, please submit an additional monthly Early 
Lapse Protection Premium for each month 
from the Policy Year Date to the issue date). 
Payment will only be accepted by cheque or 
payment by wire (telegraphic) service drawn from 
a Hong Kong or Macau bank account of 
the policyowner only. No additional premium will 
be accepted until the policy has been 
issued.
