USER GUIDE 核保指引使用指南
An impairment can be found either by ’alphabetical order’ or by ’human system. ’病患资料可根据疾病名称的首个英文字母或按人体功能系统查阅。
You can look up the usual medical impairment requirement of each illness which
 is arranged in alphabetical order under the respective human system section.  
 (阁下可从目录 按人体功能系统分类），按英文字母顺序排列之 各项病患资料， 寻找有关之医疗核保规定。
ii) for insured below age 18, please submit Original Consent & Declaration of 
custody form + patient card copy  + Parent’s ID copy, birth certificate / ID of insured.

5 Medical Questionnaire required should be completed by client rather than
 by nurse or doctor if no medical examination is needed. 
客人不需要作体格检查，客人须自行填写相关之健康问卷。
6 In order to speed up the underwriting process, please base the following 
list of questions to collect sufficient information of client’s health history. 
请参照以下问题，取得客户之充足病历资料，以便加快核保过程。
a) Date of onset/attack 首次出现病征的日期?
b) Details of attacks 病发的详细资料:
- Duration of each attack 每次发病期多久?Frequency 发病率?
- Severity 病情?Date of last attack 最后一次病发日期?  
- Condition since last attack 由病发日起至现时的状况?
c) Name & address of doctor’s proposed insured consulted  诊治医生的姓名及地址?
d) Diagnosis 诊断结果?
e) Investigation & results 检查及结果?
f) Type of treatment治疗种类? Still under treatment 是否仍在治疗中?
g) Any plan to under go surgical treatment or other treatment in the future  是否计划或 被建议接受手术或其它治疗?
h) Last consultation date 最后诊治日期? Current condition 现在情况?
